A digital Health Intake form and Public Health Screening form will be sent via email or text along with your appointment confirmation. Your Digital Intake Form should be used to make your therapist aware of your session goals and any pre-existing concerns. Your Public Health Screen reviews vulnerability to illness, symptoms & exposure.
