If you would like to request reimbursement from your insurance company* or request session notes for your HSA, we will gladly provide you with a detailed, coded invoice for your session. Simply ask your therapist after your appointment, or email intuitivewellness@icloud.com with your request. Please allow 4 business days for us to produce the documentation for you.
*If you are requesting insurance reimbursement, your provider will likely need a diagnosis code. We cannot provide this for you; it must come from a doctor. We will gladly provide you with a diagnosis referral form that your physician can complete.
